Organizations desiring to host a tournament in the Affiliate must apply for a USA Hockey sanction number. This sanction is required to demonstrate that the tournament organizer is duly registered with USA Hockey, that ice has been acquired, rules have been established and that USA Hockey insurance is in force during tournament games. Teams traveling will be required to include the sanction number on their Travel Permit requests. Tournament organizers should follow these steps in submitting their Tournament Application:
1. Complete each application separately (if multiple tournaments).

3. Email one copy of the tournament rules. Make sure they include:
- Tiebreaker rules
- Credentialing process
- Individual responsible for check in procedure
4. Send one check, made out to USA Hockey in an amount equal to $50 ($75 if Canadian teams will be attending) for each tournament application
5. Send one check, made out to TAHA, in an amount equal to $25 for each tournament application (indicate the dates of the tournaments on the check)

6. TAHA will review each application, assign a sanction number and forward with check to District Registrar.
7. District Registrar will approve, sign and mail the form back to Darla or Bettepat.
8. TAHA will distribute approved application to tournament organizer
